Overview
The Nameless Hospital is one of the most challenging anomaly commission puzzles in Neverness to Everness. Players need to find 3 jigsaw pieces scattered throughout the hospital, place them on a table to unlock the bathroom, and then navigate through additional rooms including a library-style area. Many players get stuck after placing the jigsaw pieces.
Prerequisites
- Reach Hunter Level 20+
- Accept the Nameless Hospital commission from the commission board
- Recommended team level: 22+ (enemies are Level 22-25)
Jigsaw Piece Locations
Piece #1: Reception Area
Located on the reception desk on the ground floor. It’s partially hidden behind a stack of papers. Interact with the glowing spot on the desk to pick it up.
Piece #2: Second Floor Ward
Go up the stairs to the second floor. Enter the third room on the left (the ward with the broken window). The jigsaw piece is on the bedside table next to the patient bed closest to the window.
Piece #3: Basement Storage
Take the stairs down to the basement. Navigate through the dark corridor (use your flashlight) and enter the storage room at the end. The piece is on a shelf near the back wall, next to some medical supplies.
Placing the Jigsaw Pieces
Once you have all 3 pieces, return to the main lobby on the ground floor. There’s a table with a partially completed jigsaw puzzle. Interact with it to place your pieces. This unlocks the bathroom door that was previously locked.
After the Bathroom
This is where most players get stuck. After entering the bathroom:
- Interact with the mirror — this triggers a world shift (similar to the light/dark mechanic)
- Exit the bathroom — you’ll notice the hospital has changed
- Go through the library-looking room — it’s the large room past the reception area that was previously inaccessible
- Find 2 more rooms past the library:
- Room A (Operating Theater): Contains a boss enemy and a key item
- Room B (Director’s Office): Contains the final puzzle solution
Library Room Puzzle
The library room has bookshelves with colored books that need to be arranged in order. The correct sequence (left to right) is:
- Red
- Blue
- Green
- Yellow
- White
Pull the books in this order to unlock the passage to the next rooms.
Boss Fight: The Surgeon
In the Operating Theater, you’ll face The Surgeon, a Level 25 anomaly boss.
- Element: Psyche
- Weakness: Cosmos attacks
- Key Mechanic: Summons phantom scalpels that track you — dodge sideways, not backward
- Phase 2: Becomes invisible periodically. Use your scan ability to reveal his location
- Tip: Stay in the center of the room to have maximum dodge space
Final Puzzle: Director’s Office
After defeating The Surgeon, enter the Director’s Office. You’ll find a combination lock on the safe. The combination is found by reading the patient files scattered throughout the hospital:
- First digit: Number of patients in Ward A (check the clipboard in the second-floor ward) = 4
- Second digit: The doctor’s office number (on the nameplate outside) = 7
- Third digit: The year the hospital closed (on the newspaper in reception) = 3 (last digit of 2013)
Combination: 4-7-3
Open the safe to collect the anomaly core and complete the commission.

Tips
- The hospital is dark — adjust your brightness settings if you’re having trouble seeing
- All jigsaw pieces glow faintly in the dark; look for the subtle blue shimmer
- If you die to The Surgeon, you respawn outside the Operating Theater (you don’t lose progress)
- The combination lock has no penalty for wrong attempts; you can brute-force it if needed
